What is the HealthShare Partnership?
The BVHC model of care is based on shared responsibility. We don’t provide charity or free care, but provide affordable care for a reasonable fee based on an individual’s ability to pay for the services.
Bighorn Valley Health Center offers membership in the HealthShare Partnership (HSP) to individuals who have a total household income that is less than 200% of the current poverty level. If you qualify for HSP membership, you will have access to discounted fees for service.
What does HSP assist you with?
What is the Income Chart?
The Income Chart is a formula used by Bighorn Valley Health Center to determine if you qualify for assistance.
HOW TO APPLY FOR THE HEALTHSHARE PARTNERSHIP
Click here to print the HSP application. Fill it out and send/bring it in with proof of income.
Click here to view the income chart used to determine if you are eligible for membership in the HSP.
Drop by one of our clinics at 10 4th Street West, Hardin, or 501 Main Street, Ashland. Or you can call us in Hardin at (406) 665-4103, or in Ashland at (406) 784-2346 and ask for an application. Fill it out and send/bring it in with proof of income.
Proof of income can be the first page of your most recent income tax statement - we use the Adjusted Gross Income at the bottom right of the page. Or you can use your last three pay stubs, your last three bank statements, or your annual Social Security Statement. Ask us if you are uncertain about how to document your income.
* Your HSP enrollment covers all family members in your household for one year from the application date and must be renewed annually.
If you have health insurance but meet the income guidelines for the HSP, we will bill your insurance first, and discount the remaining unpaid charges according to the HSP income chart.
If you feel you cannot afford your co-pay or charges, our team will work with you to set up payment arrangements. Call (406) 665-4103. We will not refuse you care because you can’t pay.
