What is PCMH?
Patient Centered Medical Home represents an idea; that the patient is the most important person in health care.
Nobody knows more than the patient about his or her own needs and goals. Health care professionals can assess and advise, but people are much more likely to reach their goal if they work as part of a team with their provider(s). This means that patients who choose to actively participate in discussing their concerns and issues, decide to learn what's involved in pursuing wellness and then take steps to actually move toward health are far more likely to experience success.
Every PCMH-recognized practice must demonstrate that it is built on a standard of excellence in pursuing the core ideals which are essential to the health of the whole individual. These ideals are: Being COMPREHENSIVE, remaining PATIENT-CENTERED, services which are COORDINATED and proven ACCESSIBILITY.
Total health requires attention to every part of a person's life. Many people assume that being healthy simply involves making a medical appointment when something is wrong, but in reality, people are complex. The health of the body affects emotional health, and vice versa. Chronic pain often has many various effects which a patient may not recognize at first, and preventive care is essential for sustaining wellness and avoiding preventable illnesses. Approved centers like BVHC use a variety of methods and approaches in order to take everything into account to promote health in every area of life.
Medical homes recognize that patients are partners in their health. This might be the most significant element of the PCMH approach. People need to understand what health involves, what steps are required to reach their goals and how they can be involved in the decision-making process. Without that kind of involvement, health care can become one-sided and ineffective. That's why PCMH providers respect patients as the most important team member in the journey toward health.
Total Health Care requires the best in every area. Since health problems may send seep down into any number of unseen areas (affecting emotional wellness as well as physical) it is essential to look at the big picutre of a patient's health rather than merely examining single, specific symptoms. At our Health Center we use teams of providers, each with their own specialty, who work together with people in order "wrap-around" every patient, making a complete strategy for health and wholeness. You will meet a whole team of care givers that will work with your primary provider to help you understand your own needs and what you can do.
One essential component of approved Medical Homes is Accessibility. After all, your succes depends on being able to connect with your team. We know that your schedule is important to you, so we keep our Health Center open longer hours and on Saturdays in order to serve our patients and our community.
PCMH qualification is a rigorous process. Whenever a center achieves that status, it means they have established high standards, along with ways of measuring patient success and satisfaction. Because BVHC has reached the goal of meeting that qualification we'll never have to guess at what works; our patients' satisfaction and progress are measured and monitored for results. We'll constantly be looking for what works best for our patients. We're only successful when our patients are.
